He was told he needed gum treatment for his gum disease We do pre-treatment for all of our patients and inform them of all procedures and cost We do not have on file his Blue Cross insurance information He agreed to the first part of the treatment after reviewing our pre-treatment form and signed the form We did inform him we our of his network for his insurance We did try to submit to this insurance company I called him and informed me we resubmitted everything to his insurance and offered him a discount off this procedures I told him if he could pay $we would settle this claim He told me he was not going to pay anything and we should have told him he was out of network I explained to him that would be rude on our part to immediately tell a patient they are out of network and this is the patients responsibility to know who is in network and who is not Then he accused of us of copying and paste his signature on a treatment plan I informed him we do not do that and that he signed the form He was very rude on the phone He then came to our office and again I offered to settle the claim for $and he again told us he was not going to pay anything I then told him he would be sent to collections He asked that the dentist call him which the dentist did The dentist clarified that we offered him a discount and did he want to accept He refused the offer The dentist told him we could not write off

I was told that I needed a gum treatment for gum disease and was told by the dental office before my treatment that it would be covered by a combination of my Federal GEHA Dental insurance and my Federal BCBS Primary health insuranceWhile under the impression that I would not incur any out of pocket expense for the procedure I went ahead and had the first part of the treatmentI then received a bill from Advanced Dental that stated that I had a balance forward of $1700+ with no explanation of the said charges I immediately contacted the dental office and at that time it was explained to me that several of their patients charges had been improperly sent to their respective insurance companies listing the dental hygienist on the bill performing the procedure rather than the DoctorI was told that my bill would be resubmitted to the insurance company and I would not have to pay that billOver a month later once again I received yet another bill for $1760+ At that point I contacte

He was told he needed gum treatment for his gum disease.  We do pre-treatment for all of our patients and inform them of all procedures and cost.  We do not have on file his Blue Cross insurance information.   He agreed to the first part of the treatment after reviewing our...

pre-treatment form and signed the form.  We did inform him we our of his network for his insurance.  We did try to submit to this insurance company.   I called him and informed me we resubmitted everything to his insurance and offered him a discount off this procedures.  I told him if he could pay $800 we would settle this claim.  He told me he was not going to pay anything and we should have told him he was out of network.  I explained to him that would be rude on our part to immediately tell a patient they are out of network and this is the patients responsibility to know who is in network and who is not.  Then he accused of us of copying and paste his signature on a treatment plan.  I informed him we do not do that and that he signed the form.  He was very rude on the phone.  He then came to our office and again I offered to settle the claim for $800 and he again told us he was not going to pay anything.  I then told him he would be sent to collections.  He asked that the dentist call him which the dentist did.  The dentist clarified that we offered him a discount and did he want to accept.  He refused the offer.  The dentist told him we could not write off.

[redacted],

This letter is in regards to [redacted] complaint at our office, A Dental Spot. He stated that we never told him the correct amount for his crown #30. Patient came in with prior crown broken and told us he wanted the same crown which was a gold crown. Dr [redacted] had stated to him ok we would charge the same price as we did 2 years ago which was $1200 when it was originally placed since he has been a long time patient. We also let the patient know that if it were to be a porcelain crown it would be $900, which he stated he wanted the gold. He signed a consent that he was satisfied with crown when we cemented on tooth #30. We prepped the crown on 06/28/2013 and we cemented final crown 09/16/2013. Dr [redacted] cemented the crown and once he was finished he had handed front office $400 cash and his Visa to charge $800, then suddenly [redacted] reached over the counter and grabbed the cash off the desk and he said he was only paying $800 with his visa, then told us to take him to court and walked out slamming the door.

Sincerely,

Review: I went to [redacted] to have my teeth cleaned and on that visit he told me I needed two root canals and two crowns on both upper back molars. So I scheduled the appts. and had the work done and paid for. I had pain in both teeth that would not go away it kept getting worse, he took x-rays and more x-rays of my teeth and told me he did not see anything wrong with them and just prescribed me more antibiotics and when I finished all those it was still hurting where I could not chew or eat so I went to another Dentist for a second opinion. [redacted] found with one x-ray that both teeth were very infected and he sent me to an Endodontics Specialist in Post Falls, Id. [redacted] did his consultation and he found that my upper left molar was so infected it could not be saved it had to be extracted with a bone graph put in and the other molar the upper right had to be retreated, I have spent over $8,000 fixing these two teeth (tooth #3 #14) [redacted] ruined. [redacted] kept telling me he would reimburse me and fix my teeth like they should of been but its been over a year almost two years since then and I yet to hear from him or receive anything from him to make this right. My teeth mean a lot to me and I have never had problems with my teeth until all of this. I think [redacted] should be held accountable for his actions!Desired Settlement: I would like to be reimbursed the money I had to pay out of pocket to fix this besides what insurance paid. The cost of the extraction, bone graph, molar retreat, molar implant, wisdom tooth extraction (which was pushed down due to the infection) and the gas and time it took me to go to all these other dentist and specialist. I'm only asking to be reimbursed the amount of $4700.00.
