This letter is in response to a complaint, ID ***, filed on 9/24/against our dental business. Our patient was provided detailed treatment plans for all proposed dental work prior to beginning any treatmentThese plans listed fees for each procedure and were accepted, signed and dated
by the patientThe patient actually had anterior crowns placed rather than veneers as she stated in her complaint but the remaining balance she listed for crowns was correctWhat the patient failed to mention was the "in-house" insurance plan we offered and she purchased and agreed to pay for monthlyAlso not mentioned was treatment she had on May 7, 2015, consisting of an extraction, bone graft, tissue barrier, and surgical placement of a dental implantPatient was continuing to make payments, as per her request, on this treatment as well as the crowns and in-house insuranceAll payments that had been made up to that point were made in person with patient's signature as authorization. Patient also disputed these charges with her credit card companyThe credit card company investigated her complaint and ruled in our favor due to signed consent formsPatient has since canceled all credit cards and has a remaining balance with us in excess of $500.00 Sincerely, Vladimir Khramoy, D.D.S

*** *** I apologize for the delayed response.as my computer was hijacked rendering it as un-useable I think I need to forward to you copies of what I'm working with I found even more charges since our first contact.here is what i'm looking atI am satisfied with the work just way over billedi requested my invoices be paid on different cards and I would pay a larger chunk up frontI have a contract stating I will pay roughly dollars for three consecutive monthsand that it will NOT exceed $2500.I was in the process of moving state to state while getting two off to college and did not notice the disparaty of which they sent two additional billsa couple hundred eachI had to go back and was astonishedi contacted them immediately for my invoices of what they sent 2: believe they were $and $300. I phoned them again for all of the charges for additional invoices coinsiding with chargesi told him I simply wished to cross referencing invoices against my actual chargesi received nothing.I don't think a case should close when the facts have not been examinedi have no idea what feedback you've received and I can look at invoices actually signed by me and I being able to look at the to say yes that is my handwriting the case should not haver been closed I am looking for you to reopen the case and I request that both you and I receive invoices with signature for each and every invoice I'm hoping you will cooperate with me on this hopefully last venture in order to satisfactorily put this to rest.please don't hesitate to phone*** **. *** ***

Re: Complaint [redacted]The patient began Invisalign treatment with our office 6/23/2015. The patient had an agreement to begin treatment with the office for $4500.  This price that was agreed upon did not include retainers and the patient agreed to proceed with the treatment.

To Whom It May Concern:
Patient presented to our office 10/30/15 with one of our advertisements offering a dental cleaning, exam and x-rays for $49. Patient also provided us with her dental insurance information. X-rays were taken and Dr. Khramoy examined the patient and discussed her oral hygiene...

extensively, recommending she have a debridement (deeper preliminary cleaning) as a prophylaxis, or standard cleaning, will not be sufficient due to patient's current dental/periodontal condition. Our hygienist, who was new to this office, set the appointment complete which automatically entered x-rays, cleaning, and exam into the patient's ledger. This was mistakenly sent to the patient's insurance. When the error was realized, a new claim was sent to the insurance stating a cleaning was not performed. No payment was received from the patient.
The insurance company did not receive our revised claim and on December 2, 2015 a payment for $85. was received from the insurance company. $51. was paid for the cleaning and has been returned to the insurance company.
Patient did schedule an appointment for debridement 11/13/15, but canceled the appointment 11/11/15 stating she burned herself severely but would call back to reschedule.
