Dear ***, Thank you for contacting our office on behalf of *** ***. Endodontic services were rendered by our office ranging from September-November 2016. After completion of endodontic treatment, it was determined that *** tooth would be best treated by extraction
with an implant replacement. Upon learning of her additional treatment at a different provider’s office, a refund was issued in our accounting software on 2/20/2017, but the actual credit never went through the merchant processor. I was unaware of her attempt to contact me in March 2017, which stemmed from a miscommunication with my front office staff. It was never our intent to withhold any funds from *** for her services at our office; it was simply an error on our part. I have contacted ***, both with phone communication and a written letter, and have written her a check in the full amount of $she paid to our office for services rendered (please see attached copy of letter). I sincerely apologize for this oversight on our behalf and any inconvenience this has caused ***.
