(including any family members on account and or responsible party members) if the HIPPA compliance form is signed. Once this document is signed by all parties involved in the complaint Advanced Family Dental will release all documents necessary to resolve the complaint. In this particular case we may only be able to release financial information that may include but not be limited to collections letters, aged accounts receivables, and payment history. thank you!
