On May 2, 2018, I met with Ms. N and Dr. S employees at the dental office of Advanced Family Smiles, to discuss a dental plan for my teeth. According to the agreement drawn up by Ms. N and approved by Dr. S the crowns and bridges would cost $13,000, and would need to be paid in cash (checks and credit cards not accepted). In addition, I was told I would receive all payments made by ***.

I believe the agreement for these procedures is now fraudulent for the following reasons:

• After paying $9,269, I requested an itemized bill twice for the remaining balance. I was told that I “would not understand it,” that I owed the amount on the bill they sent me ($4,647.60), and no further explanation was given.
• I never received the insurance payments from *** as promised by Ms. N, nor was I ever told that the office had received any payments. (See enclosure 3.)
• It appears that the office is knowingly and fraudulently double-billing both the client and the provider for the same services—which may be the reason they only accept cash payments. (See enclosure 3.)
• It is unclear how many crowns and bridges were actually received, and one bridge has
come out twice. Moreover, they refuse to cement it in permanently until the balance is paid and have also informed me that this bridge will never stay in and some other procedure is now required.

In regards to the complain filed by ***. She says the procedure is fraudulent and she paid around $9269 which she believes is for all her treatment which is not true. Please see attach all signed treatment plan after which we started her treatment. She was explained everything in details and that is the reason she signed her treatment plan reports see attached. She first tried to take this matter to her insurance company which is *** Dental and after we had send them all the reports which are attached as well *** also agrees that the treatment was provided and it was all corrected. Please see attach *** reply to the letter we they had sent. We are happy to fix the issues she has by having another doctor look at the case and can redo the case for her but because she does not want to pay her reaming portion of her balance she does not want to get treated with us. She says she never received the payments from *** which is also wrong as the insurance company is going to pay their portion for what is covered which is only $1500 per year and Marilyn got work done for over $15000 as she did all her crowns from tooth number 3 to 14 and from 23 to 30. We did offer her a discount to get all this work done. She says she was suppose to receive the money from her insurance as this is also wrong because insurance company will not pay the patient they will pay the provider for the service that was provided. She says the money would need to be paid in cash which was also not true as she has paid us part of it in cash and part of it on her credit card as per her she was the one who said she will pay us in cash which is also written in the treatment that was signed by her. We are happy to provide her with all the he has requested and that is the reason she knows that she has to the balance of $4,647.80 otherwise not sure how would she know about it. She is also given the treatment plan which she signed and it has all the breakdown of it. Since she has insurance it her responsibilities DR. DUDHAT & ASSOCIATES Creating Beautiful Smiles for Generations to call the insurance company and find out what she does not understand in terms of her contract. All the work we do is been submitted to the insurance company if they have any dental insurance as that is the legal part of it and her insurance company was given everything what we have done if they felt we have done anything that was wrong or fraudulent they would have asked us to send more information. They do ask us to send what they feel is not done the correct way and once they do their research they will inform us and will inform the patients as well before us. We are doing everything digital and send all the necessary information which the insurance company requires, So there are no chances for us to do anything fraudulent as we have always followed what is required by the insurance company. The reason she complained about this is because she does not want to pay her balance for the work that was done. We also tried to help her by doing the payment plans as we do not do this because our office policy is to pay in full at the time of her treatment start date. On the treatment plan she was told that as per your insurnace company we have to send Pre-D for all the work we perform but she refused to do so and she was the one who requested us to start the work with sending it to her insurance company first to see what they will cover. We do this for all our patients who has to get this type of major work so we know how much the insurance company will cover but in this case she did not want to do it. It was highlighted and she signed on it. May be we feel she might wanted to do something wrong on her side that is why she refused to send it to her insurance for preauthorization(Pre-D). But we do not know as this was her choice and we went by what she wanted. One the treatment plan is is also explained clearly that total treatment was for $15000 as you can see she got most of her teeth crowned and insurance will pay $1500 which is her maximum per year, and the rest must be paid by her which comes down to $13000. We are happy to mark the payment in full and discount her balance if she comes to the offices and signs a release form so we do not have to be responsible for anything if it happens when she goes to another dentist. Until she does not sign the release of all claims paper from our office her balance will be kept open and may result in going collection, but we can help her of what she wants us to do is to mark it in full if she comes and signs the paper work. We are also requesting her to take this complain back as we are doing what she is asking us to do is mark the balance paid in full. We are also happy to ask us to give us a second chance by helping her fix the issues she has at no additional cost but request her to pay the balance she has once she is happy with the redo of her work that she is not happy with. We have left her a message that we are working on this and we are happy to address her concerns. We are also happy to send anything that Revdex.com wants other then what We are sending. We can also send all the X-ray that *** for her to go to a different dentist. We just want to make sure she is very happy with the work that we have done and we will try our very best to provide her with the highest quality of care and services. We also want to request Revdex.com to please take a look at this and remove our complain as we have not done anything wrong on our side as even the insurance company of *** also said we have provided her with the best highest quality of care and done everything after *** gave us the permission to do so and we are happy to still help her with all her concers.
