Failure by a doctor to deliver my medical records when he shut his practice down.I have been a patient of Dr. [redacted] for a couple of years and last November I was informed that he was shutting his practice down. If I wanted my medical records, they indicated I needed to send them $15. I paid them with credit card and it was billed to my credit card. I never received my records. I called the office but the phone number was disconnected and the office was empty. I now have no medical records and other doctors won't continue treatment until I have medical records so I am desperate. Please help me!
