Failure to file the insurance company and failure to follow their guidelines. Instead I received a collection notice and I paid $27.96 with [redacted].Date of Doctor's office visit: 4/23/2013Patient Name: [redacted]Account # [redacted]I have corresponded with the business both by telephone and by regular mail and did not receive any acknowledgment of my letters. They failed to explain why I should pay the amount. My insurance informed me that I owed nothing to the business.In order to protect my excellent credit rating, I just paid the amount.I received a threatening letter dated 5/29/2015 and I paid $27.96 as directed by the business using my [redacted] credit card. I have a receipt for the payment. Desired SettlementI have a primary insurance and a supplemental secondary insurance. The secondary insurance pays whatever not paid by the primary and consequently with 0 payment by me. I have had treatment with more than 5 doctors and hospitals and labs but I did not face this situation ever before. In spite of my repeated letters and providing the information on how to file the insurances, the business failed to follow and asked me to pay the unfair amount.
