We followed through and emailed Mrs [redacted] the form to request her medical records.? She filled it out promptly and emailed it back to our clinic.? We immediately gathered her medical records and mailed everything out to her yesterday, 9/26/17.? Regarding her seeking out other chiropractic care, we were unaware of this.? She has every right to do so and we never had any issue with this.? She chose to continue to come to our clinic for care and we gave her the best treatment that we offer within the scope of what we do.? [redacted] only left us one voicemail on this matter, and that was to inform us that she was looking to go elsewhere for care, so we are not sure what she is referring to when she states "despite my voicemail(s).? We were completely surprised when Mrs*** contacted us to demand that we pay her $so she could continue care at another chiropractor's office. *** was a patient of ours from 2014-2015, and returned for care in 2017. She never vocalized any complaints that she was
dissatisfied with her care. She came to our office for a total of visits beginning on 1/9/17. She utilized both our chiropractic and massage services. When we went over her plan of care on 1/10/17, we recommended a specific care plan. All services and units to be charged were listed on that plan, of which she signed. It was never stated that she would be covered for visits a week for the entire year. Verification of her benefits allowed for chiropractic and physical therapy visits. Her initial care plan was for visits upon which she continued care beyond that period of time. We have notes and a ledger that supports all of her visits and we would be more than happy to provide those to her. We do however have a protocol for that. She would need to come in and fill out a request for medical records, as is standard in all medical clinics. Thank you, Dr*** ***

I am rejecting this response because: I was surprised by their response, they knew I was seeking other chiropractic care and that I was in a certain medical condition that they admitted they do not treatI have spoken with the staff on looking for a chiropractor, even though it was not Dr***They also stated that I could be seen 2x week for weeksDespite my voicemails, this is the first time that they offered me to get my records, if they can mail or email me the form as I had to get a job to pay for their misinformationI would also prefer not to go to in and have any and all communications in writingI have asked for these records to confirm all the visits and they were radio silentIf and when this Revdex.com complaint is closed, I will follow up with the business with an invoice and proceed with this claim to the fullest extent possible as provided by the law.

We were completely surprised when Mrs. [redacted] contacted us to demand that we pay her $1500 so she could continue care at another chiropractor's office.  [redacted] was a patient of ours from 2014-2015, and returned for care in 2017.  She never vocalized any complaints that she was...

dissatisfied with her care.  She came to our office for a total of 34 visits beginning on 1/9/17.  She utilized both our chiropractic and massage services.  When we went over her plan of care on 1/10/17, we recommended a specific care plan.  All services and units to be charged were listed on that plan, of which she signed.  It was never stated that she would be covered for 2 visits a week for the entire year.  Verification of her benefits allowed for 30 chiropractic and 50 physical therapy visits.  Her initial care plan was for 24 visits upon which she continued care beyond that period of time.  We have notes and a ledger that supports all of her visits and we would be more than happy to provide those to her.  We do however have a protocol for that.  She would need to come in and fill out a request for medical records, as is standard in all medical clinics.  Thank you, Dr. [redacted]
