I spoke to the patients mother on 12/14/When speaking to the patients mother explanation was provided where the payments were located and promise to mail hard copies of all itemized bills would be mailed to her once the payment of $made was located and posted to the patients accountAs promised the itemized bills were mailed to the [redacted] 12/17/eveningPlease allow 3-business days for the itemized bills you requested to be receivedHard copies have been mailed Thank you,

I have spoken to the patient and I have explained the payments in dispute are posted to the patients account and dates of service disputed have also been addressed I have sent itemized bills to the patients parent to show where the payments have been applied to I spoke to the the patients mother 12/15/ Thank you,

I spoke to the patients mother on 12/14/2015. When speaking to the patients mother explanation was provided where the payments were located and promise to mail hard copies of all itemized bills would be mailed to her once the payment of $100.00 made was located and posted to the patients account. As promised the itemized bills were mailed to the [redacted] 12/17/2015 evening. Please allow 3-5 business days for the itemized bills you requested to be received. Hard copies have been mailed.
