Review: AFA Plumbing did some work for the Ethical Society (EHS) - in March 2012 - removed 2 gas fired hot water boilers and replaced with new ones. Reused existing pumps and controls, etc. The EHS was told that there remained work to be done but we were not financially able to order that work at that time. We subsequently contact AFA in July 2013 to commence the work. We received an estimate from [redacted] on July **, 2013 that he would install 2 smaller boilers rather than a large residential one and that the cost would be $1200. This work was a promised for the 2nd or 3rd week of August. Repeated phone calls were made to [redacted] asking when the work would be done, and in an email from AFA, we were told that the job would be done by October *, 2013. The job has still not be initiated. A member of our Board of Trustees at EHS, a lawyer wrote to [redacted] telling him that he had a deadline of November [redacted] to do the work. [redacted] was called several times the week of Nov. ** to make sure he had received the letter; he said he had but did not bother looking at it. The secretary confirmed that it had been received. Messages were also left and still no response. We have begun soliciting quotes for the outstanding work and in our lawyer's letter to [redacted], we made it clear that he will be responsible for the cost of the repair. We have a running trail of emails from [redacted] documenting his intention to do the work.....,Desired Settlement: If [redacted] cannot do the work, we expect to have another contractor install the needed boiler(s) and will bill [redacted] for the completed job.

Review: This contractor installed a hot-water radiant-heat system in our sunroom (approximately August, 2012).

The system performed poorly the first winter: the temperature was never even or constant. The plumber maintained that that was normal and several adjustments were needed to get it right.

This past winter, the room was freezing. We called but never received a service-call.

Two different plumbers have confirmed that the installation was done incorrectly:

1. No balancing/mixing valves were installed;

2. 1/2" pipes/tubes were used instead of 3/4";

3. The temperature sensor should have been buried in the mud in the center of the room: it was not;

4. A foam/fiberboard insulating surface should have been laid below the tubing to prevent heat loss into the ground: no such thing was included;

5. A wire mesh should have been placed above the tubing to spread the heat evenly: no such thing was included.Desired Settlement: The floor needs to be removed, the work re-done & a new floor put-down. I would prefer the work to be done by another contractor.

Review: I had an oil-to-gas conversion done for my heating boiler and hot water heater with AFA Plumbing. In the first place, they took months to get the job done which required many calls from me and various excuses for delays on their part. Once the job was finally completed, they have failed, (despite my repeated requests via phone and text message), to produce the proper paperwork and documentation for the 1000 dollars worth of rebates on the boiler. I spoke to [redacted], who one of the rebates are from, and they need the receipt for the boiler to process any rebates, which AFA has since they purchased the boiler. It has been three and a half months since the job was finished and they told me it would take about two weeks to get me the proper paperwork to get my rebates at the time.Desired Settlement: I would like to receive my rebates that we are due for the new equipment, which AFA promised to deliver, or at the very least have the proper paperwork delivered to me so that I can submit them myself.

Review: I decided to have AFA plumbing convert our oil heat system to gas heat system in November of 2012. After the heat system was converted to a gas system the companys owner stated they would service any issues with the burner for up to two years. The owner also collected up front $250.00 for a "permit" for the system which to date we are still without. After multiple attempts to contact [redacted] for service issues throughout the year we feel obligated to report we had to call another company to service our new burner due to safety concerns. There is air and water leaking from the system and there has been episodes of gas leaks from the system over the last few months.Desired Settlement: We would like the company to provide the services they have promised us and maintain and fix the problems with the burner which he installed in our home and provide a permit for the job.

Review: On 9/**/14 I contracted with AFA Plumbing to do an oil to gas conversion of my heating system and have an indirect hot water tank installed. Boiler was finally installed on 11/**/14. (Contractor had to remove gas piping that they had just recently installed due to me complaining that they had installed USED OLD PIPE removed from a previous job.)

Job is still not complete. Contractor does not return phone calls or texts. Boiler zone valve has been leaking for 12 days all over basement floor and contractor fails to respond or return phone calls and texts. When I stated my patience was running out and complained I did receive an immediate response and was told "DO WHAT YOU HAVE TO DO". I had given it another two weeks but to no avail. So here we go!!!

Contractor claims to be licensed, bonded and insured but files this job under another company's license.

MAJOR ISSUE: Contractor claims to have done hundreds of these conversions but somehow claims he was unaware that the boiler had to be ordered through the [redacted] authorized web site. Since contractor ordered the boiler direct from wholesale distributor, he cannot provide me with the required portal number/paperwork that HE IS REQUIRED to provide me for me to obtain my rebate. If he has done 100s of these conversions and failed to purchase through the required site makes me wonder..........

In addition, the following items at a minimum are still incomplete on this contracted job:

Fix leaking zone valve (hazardous condition) 12 days already,

Drain oil tank,

Remove oil tank fill port,

Install sprinkler over boiler as required by code,

Install pressure relief valve down spouts.(Extremely dangerous condition),

Replace used transformer that was substituted when original new transformer failed on installation,

Removal of old Boiler that has sat on the floor in the middle of the basement for over three months,

FINAL INSPECTION,

Sad to say that this contractor had been recommended by a friend and has been paid in full. My fault for being so trusting.Desired Settlement: I want an IMMEDIATE refund of the $655 rebate amount due. I want the job completed.

As I had said from the beginning, I fulfilled my end of the contract I am just asking for you to complete yours nothing more nothing less.

Review: AFA has been contacted as many as 10 times, but has failed to respond to our request for an itemized bill for services rendered. Purchase and installation of heating system was completed in late 2012, and full payment of $5400 was made. Our prior requests have been made via a combination of phone calls, letters and a fax.

Our most recent letter to AFA, dated December **, 2014, requested a response by. January *, 2015. No response has been received.

That letter specified what we are seeking:

1. A receipt describing work performed, the $5,400 payment made, and reflecting a zero balance.

2. A statement that the installation was completed in compliance with [redacted] code.

3. A confirmation that AFA applied (or did not apply) for a permit to complete this work.Desired Settlement: Letter responding to the 3 items listed above (receipt for work performed/paid, statement affirming code compliance, permit comment).

Agreement that AFA will respond to future related requests regarding work performed.
