maxillary and mandibular dentures. She returned the following day for post-op. 6/3/16 she failed her appointment, 6/6/16 we removed a bone spur and placed a soft reline. We explained to the patient to return around the end of July for an adjustment or hard reline. We did not hear from the patient mentioned above until 3/21/17 we removed a bone spur. We informed the patient that her insurance covered her hard reline from six months at the start of treatment and it was now past the time frame and we would need to submit for the service. 4/16/17 she called up upset about ill fitting dentures. I explained to the patient that procedures and treatment she was suppose to return for, she stated she knew that she had not followed thru as advised. 7/12/17 she returned for the hard reline in the morning and returned in the afternoon, we had difficulty fitting it over her tori and the maxillary denture was too tight. We rescheduled for 7/21/17 she came in we took the impressions for new dentures at no cost, we did a try in and took a new bite. We retried it in again pt was happy and delivered the dentures that afternoon. We did not hear or see the patient again until October. On 10/17/17 pt came in stating the top denture would not stay up we did an adjustment the denture and placed a soft liner at not cost, when the patient left she stated she was happy and the denture felt much better. We have not heard from the patient until we received a letter from the Revdex.com. We are willing to do a refund back to the state for the amount that was paid by them once we receive the dentures back as well as release the prior authorization.

I was never told I had so many months. And I have been back for the same problem numerous times. They cant seem to get them right. Yes I do want to return them. Whats the address I need to return them too. I wont drive another 45 mins one way because I have done this numerous times and im on a fixed income. So send me an address where I can mail these too. And I will have proof of when you get them and I want a refund sent to Medicaid so I can get them some where else

I am writing this letter in reference to case # [redacted],. We received a second letter. We are including the patient file information. In her file it shows where we told her on 6/6/16. For every patient we write on the top of the treatment page as to when the end is. It states the patient would like a refund to Medical. We are still willing to do that. If the patient is not going to physically bring them in and wants proof she mailed them. I would suggest that she have the USPS?UPS do a form that would require a signature and once we received them Our office will release the prior authorization and remit payment to Medicaid. She can sent them to the address noted above. I would like to thatn you in advance or your time and consideration
