This letter is in response to *** complaint on ***/2017.I've already address the issue regarding the wrong facility number on the Livescan request form, which was completed by the cus***er, in my response letter on November 16.The cus***er was given a copy of the livescan request form, but will enclose another copy per her request.As for any other issues between her conversation with ***, I cannot address it, for it's only hearsay
This letter is in response to *** complaint on 11/8/2017.On 10/24/2017, *** came in to our office with a Request for livescan form, which was completed by her.We entered the data as it is on her form to our system, took her fingerprint, D and submit to DOJ and FBI(As this is the procedure
for livescan)We then give the customer a copy of the completed form with our information and ATl number on the bottom of the page.A couple of weeks later, *** called our office and claims we made an error, so her livescan didn't processWe explained to her the procedure and asked her to bring in the rejection letterTold her that if it's our mistake, we will redo her livescan at no charge arid if it's her error with the information provided, then she would need to pay to have it redoneShe Won't come into the office, but keeps calling the office accusing that we didn't put in the right number of 8.One sort.A few days later, she finally sent over the letter via email, which states the reason why it was rejected because the facility number was incorrect, We then pulled out the Ilvescan form that she completed and clearly shows she had written down the number wrongThere was a number missingWe then informed her that and told her she can look at it herselfShe then claims she lost her copy, so we told her she can come into our office and will show her theCopy.She said she would come in, but never seen or heard back from her.I'm sorry she's out the $90, but it's clearly not our error and unfortunately, each agency have their own criteria needed to process.We can send in a copy of her form she completed, along with her email showing the correct facility number if needed,
