family medical history (e.g. [redacted]) were appropriately ordered by me. His blood and urine specimens were sent to the laboratory ([redacted]s) designated by his health insurance company ([redacted]). From my review of the correspondence sent to him by Quest it seems that his insurer ([redacted] denied payments for the laboratory services provided by [redacted]s. [redacted] requested for and received services for which he does not have insurance coverage.
