was submitted to NYSED ACCES VR with rock/push hand controls listed in Section 4 Primary Controls. The bid was awarded to AGOR Enterprises with rock/push hand controls listed as 4) Primary Controls equipment. The vehicle was ordered on 5/1/2015 and delivered from [redacted] Corporation to AGOR Enterprises on 5/8/2015. AGOR Enterprises installed the rock/push Hand Controls in accordance with the NYSED ACCESVR awarded bid on 5/15/2015 in [redacted], NY.  The vehicle was inspected by a representative from NYSED ACCESVR and [redacted] was observed to operate the vehicle in a safe manner on 6/18/2015 and turned over to [redacted] on 6/18/2015 with a balance due of $77 which was not paid. The title for the vehicle was not given to [redacted] as it was stated on AGOR Invoice [redacted] that (he title would be sent directly to Buyer by NYS DOT in 4-6weeks. [redacted] was not given the extended warranty paperwork at delivery because it had not been paid for. [redacted] was billed for the extended warranty per AGOR invoice [redacted] on 7/1/2015. [redacted] did not remit payment to AGOR Enterprises after numerous requests so on 8/11/2015 AGOR Enterprises wrote check #[redacted] for $2273.00 to Protective Asset Protection as to not have the extended warranty coverage become invalid. Over the course of the next year, AGOR Enterprises sent numerous notices to [redacted] requesting payment per AGOR invoice [redacted] for the extended warranty coverage. In October 2016, [redacted] contacted AGOR Enterprises to change the Hand Controls. When asked what type of hand controls he wanted installed, he stated push/pull type. On 10/27/2016, AGOR Enterprises changed the hand control type as requested by [redacted] and he paid the amount due for the extended warranty. Sincerely, AGOR Enterprises
