I had a complete nightmare experience here to the point where I believe the person practicing dentistry here could not possibly be licensed and there is no way the filthy office is in line with whatever codes are in place for medical facilitiesI am looking into how to file a complaint with the health department
In response to your inquiry dated May *, 2014, in connection with the referenced complaint, please be advised as follows:
In her complaint, the Patient erroneously claims that she had “paid in full” for dental services received at our office. In fact, she
still has outstanding balance of which she had been made aware on numerous occasions
To expedite commencement of her treatment, the Patient originally agreed to pay all costs for the work out-of-pocket and then seek possible reimbursement from her insurance. The Patient was consistently advised that our office would gladly provide the documents for possible reimbursement to her insurance carrier as soon as the Patient arranges for payment for services rendered to her
*** *** *** is a good person and a good patient. We have never had any issues in the past with her. However, we have reasons to believe, that currently she is being misguided by her daughter. Regardless of that, however, our office is willing to work with the Patient to resolve her outstanding balance and provide the necessary information to the insurance carrier

Review: I have recently have dental work done by [redacted] at [redacted]. He demanded that I pay in advance in cash, promising me that he will send out all the nessesary forms to my dental insurance which is DC37 (Office number ###-###-####). The doctor send out a form for an estimate to the insurance company claim # [redacted]. Once the work was done he asked me to come in and sign a paper that was to be send to DC37 for the claim of the money that was to be send to me. [redacted] never send the paper. I called my insurance numerous times, as well as the dentist office and was tried numerous lies by the dentist. Finally today he told me that I should stop calling him tjat he did not care about anybodys money but his own and hang up on me. The insurance compant said that they can not pay for the claim without the dentist form.I am at a loss wht to do. Please help me.Thank you.[redacted]Desired Settlement: I would like the dentist office to send the required paperwork to the insurance company, wich would reqire them to pay me directly, as I already paid in full for all the dentist work preformed.
