Hello,Apparently the information we previouly had on the original complaint was on another account for a separate child ([redacted]) which does not have a balance. In reviewing the statement sent, it is under patient ([redacted]) that has the same head of household. Several statements were sent to the address on file with no response was ever received. This balance is from a date of service from 8/17/12 for services not cover by the insurance company.Thank you,
