Dear *** ***Please accept this response on behalf of Alaris Health at Riverton, located at *** *** *** Rahway, NJ, regarding ID# *** *** *** was admitted to our skilled nursing facility from *** *** *** *** on *** **, *** as a private-pay
patient for post-acute level of careWhile *** *** spent four days at the hospital prior to her arrival to our facility, she did not meet *** criteria for coverage at a skilled nursing facilityShe was hospitalized from *** ** *** until *** *** *** under what is called “observation services” or “observation status”, in which *** considers the stay as outpatient care*** guidelines stipulate a 3-day inpatient hospital stay as needed to cover a patient’s skilled nursing facility (SNF) stay; this does not include outpatient or observation status level of careKindly refer to pp 2-of Exhibit of this response, (an excerpt taken from *** website directly on who is eligible for skilled nursing facility care) *** *** *** and her daughter were informed by both the hospital social services department as well as our admissions office that her stay would not be covered by *** Her daughter agreed and provided our facility with a check in the amount of $5,to cover her mother’s 14-day stay for post-acute care at our skilled nursing facility*** *** was safely discharged home under the follcare of *** *** *** *** on *** *** ***. In the recent follcalls from *** ***, where she requested we fill out documentation forms for *** for her reimbursement, our central business office reminded *** *** that despite filling out any forms that *** would not be able to reimburse her because she did not meet *** requirements for coverage for her stay. What we recommend *** *** do is re-send us the form, via email at *** or via fax at *** so we may file it with *** directly, although we are confident *** will deny the claim/request being she did not meet criteria for coverage.
