I have reviewed the response made by the business in reference to complaint I* *** and have determined that this proposed action would not resolve my complaint. For your reference, details of the offer I reviewed appear below. I'm in contact with my *** ***. The records showing are not matching with DrSkaff's statements billing and paymentsAlso, my services have not been completed.I would like to request an itemized statement from the billing department concerning my services on this calendar year of 2017.

*** *** was last seen in our office on Mar21, to cement a crown on her upper left cuspid that has nothing to do with a removable partial or fixed bridge.Due to her refusal to see a periodontist and her advanced periodontitis condition, her lower loose anterior (front teeth) were
extracted on May 13, 2016, and an immediate lower interim partial was inserted on that same day, (her explanation of benefits from the insurance company confirms that)She did not complain about any blisters on the tongue or the partialAs a matter of fact she was not seen in the office again until Nov10, when she had another emergency that needed immediate attention and a root canal.On Jan30, she decided to have her lower removable partial replaced with a fixed bridge The bridge was tried in on Feb, 21, 2017and she was pleased with the fit, bite and esthetics of the bridge and she consented to have it cementedThe receptionist told her that her insurance was not going to pay for the bridge since they paid for a removable partial, she was asked to pay $5000.00; however she could only pay $With her insistence the insurance was re-billed twice with photos and explanation for the fixed bridge, they still denied payment.When she returned on Mar3, for another emergency (lost veneer), she did not complain about the bridge and she was completely satisfiedHer last visit was on Mar21, to cement a crown.We did not hear from *** *** until after Aug1st, 2017, When she was turned over for collection on the remaining balance which she was being billed for every monthWe had to turn over all our accounts receivables because we had to close the office; patients did not have to pay any additional charges.On July 21, 2017, I had to close the office and I could not find another dentist to resume the operation of my practiceA note was left on the front door and on the answering machine requesting patients with dental emergency or in need of dental treatment to seek treatment with another dentist and they can forward a written request for a copy of their records and radiographs*** *** requested a copy of her records on Aug28, 2017.As you can see from the records, *** *** had a lot of opportunity to complain about any treatment between Jan30, and July 21, 2017, about her partial, Bridge or blisters or anything else. She has also complained to the *** *** office (a copy is enclosed).It is obvious that her complaint is not about the dental work (unless she has a proof from another dentist that the work is substandard), it is her concern about her credit since the account was turned over for collection.I have contacted the collection agency and requested that her account be placed on hold until this matter is resolved.I cannot be reached at the officeMy cell phone number is ***
