March 20, 2017Dear [redacted]:Thank you for your letter. Our office manager is Ellen B[redacted] - further correspondence should be directed to her at the address above.Regarding the complaint: The complainant has five children. They were followed at the [redacted] Community Health Center until late 2015...

when they transferred to our office. We have not to date received the records from [redacted] that we requested but have received their immunization records from the PA State Immunization Information System. All but the patient in question are up to date on their checkups.None of the older children have had their laboratory blood work done, despite being ordered twice and completely covered by the insurance. The laboratory is open on Saturdays,The patient in question has missed appointments on 8/1/16, 10/28/16, 12/19/16, and 1/13/17. His other siblings have missed a total of 3 appointments. There are a number of appointments that were cancelled properly by the parent.Our office policy is that as long as a patient at least calls the day before or even the morning of an appointment and leaves a message to cancel it, we do not count it as a missed appointment but rather as cancelled by the patient/parent and no penalties are assessed.On 3/3/17 our telephone triage staff wrote: "Seen at [redacted] ER for [redacted] and [redacted]. Phoned mother and scheduled ER follow up. Child using nebulizer and taking antibiotics as ordered. Mom can only come in on Thursday afternoon, no appt. available with ...that fit her schedule. Reviewed no show policy with mother and she said she would call to cancel appointment if she feels child is doing better and does not need it as she has a WCW scheduled on 3/17/17."For our part: " It is not acceptable for any employee to be inattentive to our patients and we will look into this further and take appropriateaction.On 8/24/16 our staff gave Mother instructions of how to obtain a BARTA bus pass because her car was broken.On March 22, 2017, our care-coordinator followed through with a phone call to the Mother, who apologized to her before she even had a chance to apologize. They spoke and Mother agreed to the terms for her return to AACPP. We will forward another set of lab slips to Mom tomorrow along with a list of laboratories.The child in question was brought to the office on March 17 to be rechecked. Unfortunately a checkup was not done on that day but was scheduled for his 12 month old checkup. Mom will be given a return appointment for catch up of his immunizations when he is wellOur offer for return of the family to the practice:All laboratory blood work must be accomplished on all of the older children as well as this child. We will provide her with lab slips to accomplish this. Once the blood work is completed, we will schedule the appropriate checkup appointments for all 5 children. At this time, the complainant has 4 children with one MCO and one with another MCO. We will be happy to give her instructions to change them all to the same health insurance if she desires.
