In this case, Equian’s client is *** *** *** *** (“***”) who owns and operates *** of Mecklenburg County. *** has a contractual arrangement with Equian whereby *** asks Equian to perform certain subrogation investigation services on their behalf. While Equian is NOT a provider of services, it is acting as an agent providing *** with investigative services
5. In this case, it appears that Ms *** (or someone on her behalf) responded and provided the necessary information. As a result, the file has been closed and no further contact by Equian should be necessary on this matter

With regard to this matter specifically, Trover sought basic information from Ms*** on behalf of ***. That contact was on behalf of *** (and at ***’s request). The file was investigated based on bills that indicated a beginning of treatment for thoracic spine pain. The bills began with date of service 4/10/and continue with physical therapy and last date of service 10/12/10. Ms*** responded to Trover’s discovery letters that the treatment was not related to an accident or injury and that was not work-related. As a result, Trover closed the file. No further contact related to those dates of service should be necessary.11. Trover is not a bogus company or a fraud or scam. In fact, Trover's services allow health insurance companies to be reimbursed by responsible third parties for claims paid on injuries that are the responsibility of other payors. These recoveries act to help reduce insurance premiums

Also of note, *** responded to a complaint very similar to this one registered by Mrs** in 2013.
3. In this case, Equian’s client is *** (“***”). Mrs**’s insurance is through *** *** ** ***, a division of ***. *** has a contractual arrangement with Equian whereby *** asks *** to perform subrogation investigation services on their behalf. While Equian is NOT a provider of services, it is acting as an agent providing *** with investigative services
4. The information requested by Equian from Mrs** in the letters sent is supplemental to the information that has already been filed with the original claim for coverage. While *** (and Equian) knows that medical services were provided, no information is known as to the cause of the condition necessitating the medical care or whether there might be a third par** liable for payment. Once a file is referred to Equian for handling, any information provided directly to *** by the insured is likely NOT to make it into the file (which is why despite Mrs**’s provision of information to ***, that information may not be in Equian’s file)
5. Equian is not a bogus company, or a fraud or scam. And, the requests are NOT a violation of HIPAA. Through its contractual relationship, Equian is a “Business Associate” of *** (who is a “Covered Enti**”) under HIPAA. Everything being done by *** and Equian is in full compliance with all applicable federal and state laws, including HIPAA
6. In this case, the three (3) discovery letters on this particular event (*** event #***) were sent to Mrs**. Each simply sought informationAfter no response, a fourth letter was sent by an outside law firm (*** *** ***). It appears that after that final letter, Mrs** (or someone on her behalf) responded electronically and provided the necessary information. As a result, the file has been closed and no further contact should be necessary on this matter
7. Equian is a Revdex.com Accredited Business with an A+ rating. Equian’s services allow health insurance companies to be reimbursed by responsible parties for claims paid on injuries that are the responsibili** of third parties. These recoveries act to reduce insurance premiums

In this case, Trover’s client is *** (“***”), Mr***’s health insurance carrier. *** has a contractual arrangement with Trover whereby *** asks Trover to perform subrogation investigation services on their behalfAs specifically allowed in ***’s insurance contracts with its insureds, *** is subcontracts its rights to pursue the investigation of potential subrogation claims. In this case, Trover is acting as an agent providing *** with investigative services. The information sought by Trover is information that it is entitled to request from the insured and the insured is obligated to cooperate pursuant to the terms of their insurance policy.3. Trover is not a bogus company or a fraud or scam. The inquiry is simply an attempt to gather information on behalf of his health insurance company. While *** (and Trover) knows that medical services were provided, no information is known as to the cause of the condition necessitating the medical care or whether there might be a third party liable for payment. The information requested by Trover from Mr*** in the letters sent is supplemental to the information that has already been filed with the original claim for coverage4. In this case, the first discovery letter on this particular event (*** event #***) was sent on 10/16/15, followed by additional letters, all simply seeking informationAfter receiving this complaint, we did additional investigation and determined that there was another event (*** Event #***) on Mr*** during the summer of 2014. In that case, Mr*** did respond on 8/11/that the treatment was back treatment. The treatment was not automatically tied to the injury due to lack of information. As such, it was necessary to send inquiries to get additional information in to make sure there wasn’t another incidentTrover apologizes that Mr*** may have misinterpreted our efforts to simply get additional information to ensure that the new treatment was related to the treatment.5. As Mr*** has now responded to our correspondence and provided some additional information, it is not anticipated that Trover will need any other information on the claim

In response to the Revdex.com complaint submitted on 10/12/by Ms*** *** (“Ms***”), with an assigned ID of ***, Trover Solutions, Incresponds as follows:
1. Trover Solutions, Inc(“Trover”) is a duly organized corporation under
the laws of the state of Delaware and is validly authorized to do business in Kentucky, Ohio (and most every other state in the United States).
2. Trover is an insurance subrogation company who provides subrogation related services to a number of the nation’s largest health plans. In this case, Trover was providing subrogation services to its client related to an auto accident involving Ms***.
3. It appears that Ms*** *** was driving the car of a friend, Ms***, and hit the vehicle of an insured of a Trover client. Neither Ms*** nor Ms*** had insurance on the at fault vehicle. As a result of the accident, the original amount due was $6054.41.
4. Trover representatives went back and forth with Ms*** and Ms*** as they were both responsible parties. While it appears that there was an error with the calculation of the original amount due, that error was fixed and both individuals were notified of the amount due. Both individuals agreed to pay down payments, which didn’t occur. Then they agreed to pay $down (over a 3-month period), but that also failed to occur.
5. At no point was a formal agreement executed between any of the parties here, everything was verbal and by email. To our knowledge, there have been sporadic payments between the two persons that result in approximately $4,being still due. Due to our inability to recover the sums due, this file was deemed “failure to pay” and referred to our outside collection group.
6. The sums due are valid, no consistent payments were received, and the file was assigned to and is being handled by *** (1-***) and is no longer in our control

The Revdex.com complaint goes to *** event # ***. It should be noted that *** is NOT an insurance company and is NOT a fraudulent company. *** is a vendor that performs services FOR insurance companies. In this case, ***'s client is Auto Owners. The
initial letter was sent to the responsible party on 10/27/The 3rd party, *** ***, contacted *** and stated that she (Ms ***) got out and took pictures and didn’t see any damages to the insured's vehicleMs *** requested *** send her pictures of the damages and a statement from the AO adjuster who adjusted the claim verifying damages were caused from this lossHowever, there are no photos in our file documents or in the supporting documents *** received from Auto Owners. *** did not send a suspension request to the Indiana DMV, It appears that in this state if an accident occurs and the police report notes the responsible party did not have the state required liability insurancethey will automatically suspend their driver’s license. After Ms*** refused to pay the damages, since we could not send her damage photos, the file has since moved to a outside collections vendor (*** and Associates) who has the file in active collectionsIf Ms*** doesn't have the documentation, she can contact Wilbur and receive documentation of the full amount owed (including the loss notice, the rental invoice, the damage estimate that Auto Owner's sent to *** for auditing, and their complete audit report of the damages)The total of the damages presented is $(which includes the insured’s $deductible)
