was diagnosed needing full mouth extractions and dentures, please see treatment
plan attachedThe patient consented to the treatment totaling $9,178.00, of
which his insurance was to cover $4,which is a combination of
payments
and contracted fee adjustmentsThe patient portion was $4,and the
patient paid $that day and our Dentist took impressions for his
interim upper & lower denturesThe patient was seen on two other dates for
the fabrication of his interim dentures; 5-21-14, 5-27-and 6-13-During the
process of fabricating the interim dentures, the patient had a consult with our
Oral Surgeon on 5-27-14, to prepare for the removal of teeth under IV
sedation
The patient had his teeth removed and interim dentures
delivered on 6-24-14, and he came back for adjustments on 7-3-14, 7-9-14, &
7-11-It was determined that the patient needed quadrants of alveoplasty
to help the denture sit more comfortably in the patient’s mouthOn 7-29-14,
the patient paid $( these services were not on the original treatment
plan) and the Oral Surgeon performed quads of Alveoplasty, see treatment plan
attachedThe patient was seen on 8-24-and 8-28-and we have not seen the
patient since then
The patient was to be in his interim dentures during the
healing process and after months he was to return for his final dentures
which were on the original treatment planAt that time he was to pay for them
at the time we started fabricating themSince he did not return, we did not
collect any monies for those dentures, so there are no monies that are owed to
this patient

I called Royal Dental talked to [redacted] who is a very rude man did not resolve anything.I appreciate everything that you did in helping with this problem.I and my husband are both senior citizens and if he talks to me I can just imagine how he talks to his own parents.Thank you again Kathy Fowler.
