I've attached the requested information.I let this contractor go as he was destroying my home, it had been weeks since he started and what a horrible job he did on painting and my floors in bath and kitchen weren't complete, even though he said they wereHardwood floors had to be re-done by another contractor.Almost everything had to be re-done which was more money out of my pocket.I wouldn't recommend him to anyone.ThanksLinda
