June 8, 2016To Whom it May Concern,I am writing in reference to the customer complaint about billing or collection issues.Our Statement of the Problem: We check for each patient their insurance information prior to them coming in for an appointmentWe verify via the internet website for the insurance company any co pays or deductiblesIn this instance, the insurance was checked and the online information stated a $co paymentI have attached documentation showing this information and due to privacy policy have deleted the name and identification numberThere is copy of the insurance card which states Spec $for a specialist which our office falls under and printed under the benefits itself the information regarding co paymentThe $copayment is for a primary care physician officealso included a report from the employee who dealt With this patient at the time of serviceFor added information included the processing of the first visit from the insurance company called an explanation of benefits which clearly shows that the patient is responsible for the $To this date the insurance company has not called us back and/or sent us documentation regarding this caseWe also refunded the patient the $they felt they were overcharged on that dateI would also like to address the facts regarding the appointment itself and what was doneAccording to the physician the patient did not follow any of the requests from the first visit prior to the return visitThe patient did see the physician, stating improvement and advice was given for further treatmentThe treatment the patient thought was going to happen was not needed due to his improvement.Desired Outcome/Settlement: Patient will be responsible for the portions of his co pay that was not collected on the first visit and responsible for the balance of the co pay which ended up being refunded unless in the meantime the insurance company sends us documentation that the processing of the initial claim was incorrect and the second claim processes with only a $co pay.Sincerely,Lois C.Office Manager

I have attached documentation showing this information and due to privacy policy have deleted the name and identification number.
