The patient made a copay payment that was not recorded on her billing statement. The patient left a phone message and this was not responded to promptly. We have a record that there was a response to one phone call, but the patient did not receive a phone call to clarify that her account was zero...

We are also setting a protocol to make sure that all payments are properly recorded. The consumer was notified and will be satisfied that this issue is fully resolved when she receives a written statement which shows that she has a zero balance. We did inform her that she does not have any balance at this office. This statement has been mailed.
