the electronic medical records (EMR) documentation for date of service in question support all testing completed.  Several communications were exchanged in September of 2016.  It was explained that the office visit documentation substantiated the office visit and all testing.  Due to HIPAA policies, I cannot discuss in detail the patient’s medical history.   Sincerely, Practice Manager
