Dear Sir/Madame: We are writing in response to your letter dated January **, regarding the above-reference ComplaintAlphacare by Magellan Health (“Alphacare”), a subsidiary of Magellan Health, Inc(Magellan), is a managed health care company that is responsible for the management of the

On July **, 2015 at 11:30am, when I met with one of their employees by the name of Lisa B[redacted] at my father's residence in Jamaica, Queens, NY, I provided her with the Power of Attorney papers along with various other documentation such as his marriage certificate, medicare card, SS card, SS benefits letter, rent receipts, bank statements, green card and passport.  She said she didn't need it at the time since my father was in attendance to sign, but I gave it to her anyway.  The Power of Attprney papers list both my sister (as point Power of Attorney) as well as my name (as the successor).   I also faxed the Power of Attorney papers to Lisa B[redacted] again on August **, 2015, after I was notified by Medicaid that they never received anything  from Alphacare pertaining to my father's request for Medicaid.  I have attached the Power of Attorney that was given to Lisa B[redacted] as well as faxed to Alphacare.  They did not handle the process with Medicaid properly and I ended up going to the Medicaid office and starting the whole process over myself, which delayed care for my father.
