Incompetent billing practices prohibited insurance payment within allowable timeframe. Dentist now wants payment two plus years after service.The content of this letter was sent to your office and I have continued to receive monthly bills for service over two years ago. * 6/27/12 Service received. I was insured under [redacted] Dental of [redacted]. The insurance card was provided to your office at the time of service.* 11/14/12 My wife contacted your office regarding receipt of a bill and no evidence of an EOB from [redacted] Dentist [redacted]). Your receptionist stated the claim was filed to [redacted] on 6/28/12. After verifying the correct ID number and group number with your receptionist, my wife contacted [redacted]. They had no record of the claim. [redacted] advised my wife to ask the dentist to provide an EOB or rejection letter showing what was denied and what company the denial came from. My wife contacted your office again and was told the claim had been filed to [redacted] but she would file it again.* 12/5/12 My wife contacted your office to get a status update. She asked your office to please pull the rejection letter and send to [redacted]. Your receptionist stated she had filed the claim and would re file. My wife called [redacted] and was told they still had not received a claim.* 10/3/13 My wife contacted your office as I received a bill stating that the account was 90 days overdue. Your receptionist told my wife she filed the claim on 6/28/2012, checked status on 8/22/2012, resubmitted 10/20/2012, resubmitted 11/19/2012 and on 12/5/2012, and then the claim was kicked out of the system. Your receptionist stated she would call [redacted] and get their fax number to resubmit.* 10/25/13 I received an EOB from [redacted] stating "Claims must be submitted within 12 months of the date of service to qualify for benefits. A participating dentist may not charge the patient for any portion other than co-pays, deductible or non-covered services."* 11/6/13 I received an EOB from [redacted] stating "Payment for claims received more than one year following the date the services were rendered is not a benefit of the dental plan. The enrollee was not eligible on this date of services/predetermination." This indicated that your office filed the claim with [redacted] more than a year after the services were performed. I was not covered under [redacted] when I received your service on 6/27/2012.* 5/1/2014 I received a statement from your office stating that the claim dated 6/28/2012 was submitted to [redacted] Insurance Company: [redacted] on 10/3/13 and to [redacted] on 11/6/13.o Why did your office submit a dental claim to my primary medical company, especially since I was not even covered by this insurance company at the time of service?o Why did your office submit a dental claim to [redacted] Dental of TN when I was not covered under [redacted] Dental of TN at the time of service? With all due respect, I believe we have tried to assist your office to file the claim to the correct insurance company to no avail. I respectfully ask that you cease requesting me to pay a claim that your office should have clearly been able to obtain payment for if the claim had been filed promptly and correctly.Desired SettlementStop requesting payment and send e a notice that this charge for $137 has been settled. It is not uncommon for many dental office tonot to accept 3rd insurance assignment at all. We do file claims on behalf of our patients as a courtesy. In this case our insurance manager filed multiple claims, multiple times to various insurance companies as his job changed and he got different coverages, etc. We have documentation of our 10 inquirieson his behalf to receive payment from the insurance company that we would be glad toprovide him.Enclosed is a copy of our financial agreement that he initialed when he joined the practice thathe would be billed for any services not paid by insurance in 60 days. In this case our officetried at least 10 different times to resolve the insurance over a period of 2 years withoutsuccess. The fact of the matter is, dental insurance is an agreement between the companyand the insured (the patient) and NOT with us. As a free courtesy to our patients we acceptassignment of benefits.
