explanation of benefits until it was requested by AMS. The charges were billed properly and timely to the insurance company. The insurance company applied the charges to the patient’s deductible. The balance due is patient responsibility per the insurance benefit plan. Since this complaint was filed, the patient has contacted the Patient Account Department and has worked out a payment plan to pay the balance due within 3 months.
