I received a procedure from Dr. L (no complaint of the medical care). I received the medical billing and verified the charges with my insurance Explanation of Benefits (EOB's) to ensure the billing was consistent, which it was. I paid the two separate bills with my ***.

I was subsequently billed an additional $40 and I could not get an explanation why. All the charges went through and were paid. I requested and received a copy of the accounting and found the second *** payment of $100 was received on 10/31/2019 by system user "***" as transaction #***. However, a patient refund check (check #***) was issued by system user "***" on 11/21/2019 against the account, which immediately incurred a billing statement.

I did not receive a refund check. My *** has not received a credit. I have attempted to discuss the matter with the billing department numerous times however they say they have never received payment and they do not see anything in their system. I also discussed the matter with Dr L who did not seem interested in the matter.

From my optic, I have payed everything required of me, and it seems someone with system access generated a refund check which immediately put my account into collections. It should be very easy to see who Check #*** was made out to and who ultimately cashed it, however there is no way to get any action or movement on this, other than sending me to collections. It very much seems that someone internally is creating refund checks for themselves at the expense of the patients.

I have all of the pertinent information related to this allegation in my possession.
