The date and time of this remediation class, September 15, 2015 at 8 am to 12 pm, was written down and given to her and also written in our appointment book. On September 15, 2015, Ms. [redacted] failed to show for the class. We require a 24 hour notice for a cancellation. She also failed to contact us to let us know that she was not coming. When Ms. [redacted] made the second appointment with us, it was her responsibility to keep that appointment. If she was unsure or had lost her appointment slip, she should have called us in a timely manner. Ms. [redacted] paid a total amount of $58.85.
