symptoms did not resolve the patient returned to see Dr. C[redacted] and after occlusal forces test, cracked tooth syndrome was diagnosed for tooth #2. When teeth crack it is due to occlusal forces over time.Removing decaydoes not cause the crack. Patient was told the rationale and reasons for treatment plan of a buildup and CfQ on tooth #2, and it was discussed to replace the sedative filling on tooth #3 with a composite filling.

Review: Dr [redacted] cracked my tooth during a routine filling and after telling me I would not be charged for the repair or the tooth they billed my insurance for all subsequent visits causing me to reach my maximum insurance benefit and now are telling me I would owe over $500 to place crown on the tooth they cracked.Desired Settlement: My insurance should be refundes for services I was told I would not be charged for and my tooth repaired at no cost to me.
