H was most definitely a typo error by an employee who has since been terminated for reversing her numbers when typing.We submitted a total of three bills in this short time to [redacted] in error. We have notes in our system of each one. The dates are as follows: 3/24/15, 8/10/15 and 10/24/2015.The 3/24/2015 claim was the first. We caught our error right after billing it out. We called [redacted] to advise them that we billed in error on 7/5/2015. We also submitted a letter to the patient and [redacted]. There is a note in her file at the office that was dated and mailed 7/7/2015. Please see attached.The 8/10/2015 claim was also billed in error. [redacted] from [redacted] called with [redacted] on a 3 way call on 10/8/2015. I personally talked with the patient and apologized several times to the patient. She was unhappy which is understandable. I was told by [redacted] that we did not need to send anything to them explaining our error and that my discussion with him was enough. We were never paid on this charge either. At this point noted her account Do Not Bill Patient and to please double check first.On 12/3/2015, [redacted] called and was furious with the doctor’s main office. She yelled at the receptionist and the manager, Ari, at the office. I am in complete knowledge of this account and was quite upset myself hearing this happened yet again. Then we received a call on 12/3/2015 from [redacted] and the patient. Jill M[redacted] answered the call, like [redacted] mentioned, but I was in the room at the time of the call. She was not rude in any way to the patient or the representative. She tried to apologize and explained what happened several times. Unfortunately, we did not get the representative's name. know it is hard to hear another typo has been made but that really is all it was. We were never paid on any of the claims. We did not fraudulently bill to collect or steal money. There were not eight claims submitted for this patient and the complaint of being rude is an exaggeration. Why would we be rude when we made the mistake? This doesn't make sense.We are a proud small medical billing and credentialing company that is outsourced by many physicians. We have been in business a little over 12 years now, we are licensed, bonded and have nothing to hide. Although our name, address and telephone number are on every bill sent out, we have several locations. Our main headquarters is a residence which was also explained to the patient. There was no need to give the patient this information in her rage. We have never received a complaint to my knowledge and take this very seriously. I have been in the medical billing field for well over 25 years I and attend many seminars. I am fully aware of what fraudulent billing is and this was not the case for [redacted]. We did not do this intentionally and were never paid for any service. We have apologized Several times and documentation on all three cases. Again the employee was already fired that created these errors and I am truly sorry for this.
