This is in response to a complaint describing the events that occurred at the testing center on October 5, due to a discrepancy with his middle name, as it was given to ARRT by Mr*** in our official recordHe sat for the Fluoroscopy examination previously under the rules of his state, for
that is a state examination, not an ARRT examinationIt is also the candidate's responsibility to make sure that the Candidate Status Report (CSR) and confirmation page matches the identification that you present at the testing centerMr*** was provided information about his name that is in his official record multiple times during the application processHe signed the candidate agreement that includes a statement that he read the handbook, which includes the following paragraph: If the name on your ID doesn't match your CSR, cancel the appointment and correct the discrepancy by phoning ARRT at ###-###-####, ext***Don't schedule a new appointment until you receive a new online CSR and have verified that all changes are correctAlso, in the second paragraph on his CSR, in bold, it states "Your name, as it is printed is part of your permanent ARRT recordPlease review it carefully and contact the ARRT if there are any corrections or changesNo changes will be processed at the test center." I am rejecting this response because:
By your own response and by the video on the ARRT website, the middle name is the exception to the rule as long as the first letter of the middle name matches ARRT records, which the middle name on the identification presented and the middle name on my ARRT profile both begin with an "S".
