***, [redacted] was examined times 5/6/-- was for initial vaccine, 5/14/was for an ear infection, 5/27/was for an abscess on the side of his face and to recheck his ear, 6/11/was for vaccines at which time he received a full exam and received ear treatment recommendations which included rechecking the ears at the next vaccine appointment 6/25/was for a rabies vaccine at which time he received a full exam and further treatment recommendations for his ears 7/13/he was given his final puppy vaccine and once again received a full exam which included his ears Each visit included an exam, an evaluation and a treatment recommendation Not all of them were vaccine exams Under the Wellness Plan all of those visits would have been covered by the monthly fee However, outside the wellness plan each visit would have been charged either a vaccine exam(for appointments that were primary vaccine appointments) or an office visit or recheck exam( for appointments such as ear checks or abscess checks or ear rechecks or abscess rechecks) The number of visits for [redacted] was not driven by vaccines alone, they were also primary problem visits ( Ears and abscess) The invoices are fair and accurate and what was performed at each visit is accurately detailed in the medical record Respectfully, Dominic C [redacted] DVM

I am rejecting this response because: Amigo Animal Clinic has over charged for vet immunization visitsI was told by vet that the immunization shots would be divided into separate visits instead of visitsWhen I explained I didn't understand why and felt that was for an extra cost I was told I would not be charged for the divided visitsI am disputing $for the vet visits that were not needed for a regular immunization shot visitIt was the vet who explained he "likes to do it this way." This was not a request by meIf the $charges are removed I will agree to pay the remaining balance immeadiately.

[redacted], I am sorry for the troubles you seem to be having reaching us. We are open 6 full days a week and we have always had an open door policy. Our records indicate you have been provided with all of the documents you have asked for, except for your pet's records...
which have been waiting at our reception desk for several weeks. We have attached those documents to this reply and we have sent these same documents to you via certified mail (mailed Friday last week). The MEDICAL records are quiet large so we would ask you to please pick those up at our office (they are located at the reception desk).We would welcome the opportunity to explain these documents in person. You can make an appointment with myself and our practice manager at any time. Our phone number is ###-###-####. We are open M-F 7:30-5:30 and Sat 8:00 to 5:00.
