She had an apt last year at target optical (nordin eyecare). Dr*** took over the practice and received all patient charts etc. A postcard was mailed to all former nordin eyecare patients introducing *** total eye care. This patient signed the proper documentation
allowing us (Nordin eyecare at the time of her exam) to contact her regarding future exam appts. All patients receive a pre appointment reminder for annual exam but its not an appointment until confirmed by the patient. This patient requested no further contact and was eliminated from our system the day she sent me the email. Dr***
