Assist business owners whom need to make changes within their business ex) name and/or change of ownership etc.…I would like to inform you that *** *** *** *** is the original owner of the businessOn, November **, he came to my office to register a corporation under the name of *** *** *** ***the president of the business was his sister *** *** *** *** *** *** *** then decided to change the name of the business from *** *** *** *** to *** *** *** *** *** ***Due to this change the process of filing all the required license where delayedAfter the name change was submitted to NYS Department of Corporation, *** *** *** resigned as president of the business because she couldn’t fly from Michigan where she resides to New York to file all the licenses for the business at this point we had to resubmit the Change of Officer to the Federal, State and Local Authorities to show the new responsible Officer*** *** *** *** then decided to use his younger brother whom has never filed taxes and resides in Michigan After numerous changes on behalf of *** *** we then started to have more problems with the business filing since none of the documents matched the new name of the business and president, *** *** ***The lease for Moe Prospect Deli and Grocery Incwas under *** *** *** not his brother *** *** ***We began the application for the Liquor License, we submitted the NYS Tobacco Products license, the NYC Health-Deli license and USDA-SNAP (EBT) in which the application was closed since none of the documents matched and they didn’t comply with the requirements from the USDA-SNAP (EBT)*** *** *** paid us $1,in which we paid for the following licenses (he has proof of the payments we made): NYS Cigarettes and Tobacco Products (attached is receipt) $NYC Health-Deli license (attached is receipt) $NYS Department of Corporation (change busName) $Total: $Due to *** *** *** not understanding the process of license and the lack of paper work he needed to provide in order to comply with the agencies (Bill of Sale, Lease, Personal Income Tax etc…) I decided to terminate the business relationship with himOn, April *, we reimbursed *** *** *** the amount of $941.00, which leaves a difference of $in which we took a loss on the time we spent making all these changesThis amount doesn’t cover our Service fee since they requested us to change the business name within Federal, State and Local EntitiesSince the Officers name where changed the minutes needed to be changed as wellBesides all the services we provided to *** *** *** we also helped him close a business that he previously had in New York CityThe allegations that *** *** *** *** is saying are incorrectWe have been working with him; it is not our fault that their license has been denied due to lack of proofAs an experienced person that I am the process of creating a business by filing for permits or license is very important and it all takes timeIf one document is being delayed it will affect the rest of the process for the licenses or permits for the businessIt was explained to *** *** *** *** that the process takes timeUSDA-SNAP (EBT) requested that the president, *** *** *** submit his taxes as well as proof of how the business was purchasedUnfortunately, *** *** has never filed taxes so this is one of the reasons why the process of the license has been affectedIf *** *** *** stated everything that occurred and filed for a complaint, why was the reimbursed check made payable to him instead of his brother, *** *** *** *** *** only came to our office to sign documents when needed no questions askedThe incident occurred on April *, in which we have it on video in which shows *** *** *** in the office not *** *** ***If you have any questions, please feel free to contact me at the above addressThank you in advance for your assistanceSincerely, *** ***
