Pennsylvania Victims Compensation Assistance Program.The claim forms for this program were filed in a timely manner to the *** Detective Crime Unit located at the *** *** ***Once papers are filed with the Police Department, they complete their portion of the claim form and forward the claim on to ***.On June 3, 2014, *** initiated a conformation form (copy attached) that we must fill in; sign and either fax or mailWhen I returned from vacation on June 22, 2014, (the form was not in my possession before my leaving) this form was among the many papers on my deskAs you can see from the attached fax confirmation, I replied to Crime Victims on July 1, 2014.Although *** feels that he is entitled to the monies from the Crime Victims Compensation Program, it is at the discretion of the State of Pennsylvania to approve the funds for which the State has one year or more to do so.I have not talked to *** nor have I received any e-mails from himAlso, I have not received anything from *** or the Anti-Violence Partnership of *** which is only an agency that completes the same paperwork we have already submitted to *** for compensation.All the services we were contracted to perform were handled and completed in a timely manner; therefore, no settlement is due the family from the funeral home.If any further information is needed, please contact me

Review: On May 2, 2014, we experienced a trajic loss of our father, [redacted] age 52, (D.O.B 1/28/1962)... Cause of death was murder. Our dad was an innocent bystander caught in a hail of bullets. Nix funeral home recieved his body and due to the sudden death and financial strain, we had him cremated on Friday, May 9,2014, which totalled $2,075 (that's not including cremation jewelry & urn). Due to the nature of the situation, we were eligible to recieve compensation from the Anti Violence Partnership of [redacted]/Victim Services (which we submitted May 11,2014 claim #[redacted]), but Nix's funeral home have not responded to "[redacted]" support. We've made several attempts (and spoke with a representative informing them that "[redacted]" is waiting for a respond from them. We called 4 times totalled, emailed 3 times and just recently talked to the owner which confirmed she submitted the form needed as of thursday,July 3, 2014. As of today July 10,2014@8:22 am, I am showing that they have not recieved the verification form from the funeral home. I have 3 children and a wife, I am entitled to these funds, I've made professional cries to this establishment....we've suffered very much emotionally we need Nix to follow protocol in order for us to recieve funds that's due to the family. Thank you for your time and efforts. We have no one else to turn to at this timeDesired Settlement: We would like this establishment to file the proper paperwork immediately, which is the verification form that was requested June 25,2014 via email. We have suffered enough and we are still grieving which is turning into financial strain
