*** Please accept our apologizes regarding the information given to you stating there would be No Paperwork. I am not clear if this information was pertaining to audits or just payroll, but I will reach out to Intuit to get more informationI will be sure to update you with any response that I receivePlease note that at the time you purchased your policy from AP Intego, it was disclosed to you that audits could occur and sometimes further documents need to be submitted. I have sent this signed document to you directly by email as there is personal information regarding your company that I do not want on this public site In the meantime, please let me know about Florida. Did the two employees work in Florida? As I mentioned to you earlier, we were not collecting premium as these payroll files were uploaded using a state that is not on your policy. Once I have this answer I will let the carrier know and we can work on completing this audit***~ *** I.F* Director, Customer Care Tell us why here

Mr. [redacted], We appreciate your concern of an audit bill paid in the amount of $1529.73. Please understand that we are unable to refund any money to you without completing the audit process. It appears that our audit specialist began this process with you, however, through your request, you asked...

that no further questions be asked of you which prevented us from continuing on. First, although the calculation of premium for a Workers' Compensation policy is based on actual payroll received by AP Intego from [redacted], occasionally errors and discrepancies can occur that will show-up in an audit. If the collected premium exceeds the final audited premium, then a return premium is due to the policyholder. Conversely, if the final audited premium exceeds the collected premium an additional premium is due to the insurance company. The Pay As You Go product does not remove the policy language, carrier's right, and state laws allowing insurance companies the ability to audit policyholder records to reconcile the premium collected. On 11/13/15, we collected a premium amount of $ 1529.73 which reflected an audit premium due for the policy term of 3/13/15 through 10/2/15.  Per C NA, this figure was prorated based on the prior year’s annual audit exposure.  C NA explained that if we submitted a completed payroll form providing actual wages for this policy term, they would review the estimated amount collected and revise where necessary. On 10/21/15, our audit specialist reached out to you asking that you complete a payroll form detailing the description of your operation, partners and employees. The Carrier asked that this to be completed so that they can reconcile against what payroll has been received.  A reminder was sent to you on 10/26 with no response.  The audit specialist reached out by phone and spoke to [redacted] on 10/28/15.  [redacted] asked that we send the payroll form directly to him.  11/3/15 and 11/4/15, reminder emails were sent to you and [redacted] asking for a status.  On 11/16/15 we did receive a response from you along with the completed payroll form and then 941’s.  Unfortunately, the completed form sparked additional questions from the Carrier, which involved some back and forth of emails between you and our audit specialist.  The Carrier asked additional questions which included the termination dates of previous employees, names of officers and employees working in Florida. We were not collecting premium for the employees in Florida as it was not a covered state on the policy.  We will be happy to continue to work with you or you may choose work with C NA directly.  Once these questions are addressed, C NA will review the information and revise the $1529.73 if valid. Sincerely,[redacted]Director, Customer CareAP Intego Insurance Group, LLC
