Review: I made three visits to April Rubin, beginning on November 15, 2013. I scheduled the visits after phoning the office to ensure that Dr Rubin was in "in-network" provider with my health insurance company. The administrator assured me that the office is in-network, took a photocopy of my insurance card, and collected an in-network co-pay in the amount of $25 on each of my three visits.In February 2014, I received Explanation of Benefit statements from my insurance company that indicated Dr Rubin's office is not in-network. I immediately found a new provider, and informed Dr Rubin that I was no longer her patient. She apologized for the "misunderstanding" and assured me that she would be willing to work with me regarding the medical costs that I had unwittingly accrued. On March 7, I received a bill from Dr Rubin in the amount of $920 for the three office visits.The bill was handwritten, photocopied, largely illegible, and was not itemized (it did not include procedure codes). The charges appear to conflict with/and or duplicate costs for office visits and lab work, and my account has not been credited for the co-pays that I made on each visit.I have contacted Dr Rubin's office by telephone on four occasions to request an itemized and corrected bill that can be used to compare the office's charges with those made to my insurance company, and have been informed by the receptionist that Dr Rubin is not interested in discussing the bill. I have been informed that she does not intend return my calls, nor to respond to my request for corrections/clarification of the bill. I believe that Dr Rubin's office directly misled me regarding its in-network status and is now "balance billing" me for all possible charges. I believe this is highly unethical conduct, but is probably not illegal. The bill provided to me, however, is not legible or itemized and cannot be reconciled with the charges sent to my insurance company. I believe it is the office's responsibility to provide me with a legible bill.Desired Settlement: I request Revdex.com's assistance in requesting from Dr Rubin a legible and itemized list of charges. Each charge should include a date and procedure code that can be used to cross-check charges made to the insurance company.I request Revdex.com's assistance only in obtaining a legible and coded bill that can be submitted to my insurance company's fraud department.
