Missing ***
Brought my mom here for physical therapy after an allergic reaction ***. Arrived *** in the afternoon. This is my 1st time at this. The nurse rushed me to write my moms n all her clothes and to write what clothes she had on paper. They were trying to get her room mate bathed, while I was getting mom settled in. Came to visit on the ***. Called moms cell on the ***. The *** I get a call from the day nurse, wondering if I took moms cell home with me Saturday. No, I talked to her on Sunday. Went to the facility on the *** after work to look fr her phone. Moms remote control and phone were both gone. I assumed it got picked up with the laundry. The night nurse had no idea about moms phone, stated no on told her. Spoke with *** about this and he said *** is out of town and would call me ***. No call. moms room mates daughter told me that her mom who has been there for awhile had perfume, jewelry and other things come up missing. It seemed to me as everyone kept passing the buck.

I want them to replace my moms phone.
Per Company Policy: Any item not listed on the Personal Inventory Sheet will not be reimbursed by the facility.
Daughter of roommate comments have been investigated and were unfounded.

At arrival, I was told to mark all moms clothes with permanent marker, then handed me a piece of paper and said to write all moms clothes down. That they tend to loose laundry there. Not once did Valhaven mention phones, chargers, glasses, walkers, dentures, etc. Clothes was all I was to mark down.Roomate *** grand daughter told me of all the things that came up missing while at Valhaven. I strongly feel this is their responsibility.
Our owner ***/*** and *** from *** set-up the policies/procedures for the facility to follow. I have the Inventory Checklist in front of me and it asked for the the following items: Dentures, Glasses, Hearing Aid, Medical Equipment, Glass Case, bible, Luggage, Bathroom/Hygiene Items, Razors, Books, Purses and Other items of value. Mr. filled out the form very specifically in regards to the above items, including every piece of clothing that was brought into the facility. According to the Policy it clearly states that if the item is not on the inventory list that the facility is not responsible for placement.
This is not my decision. If I were able to replace the cost of the phone I would gladly do this for Mr.. Unfortunately, ***/*** and *** are not owners that conduct business above board.
As for the other complaints of urine/feces smell, we have never had an issue of our facility having odors. We take pride in the cleanliness of our facility and to make these claims is slanderous.
To clarify 42CFR483.10 g6: The resident has the right to have reasonable access to the use of a telephone, including TTY and TDD services, and a place in the facility where calls can be made without being overheard. This includes the right to retain and use a cellular phone at the resident's own expense. *This does not mean that we are required to have a phone in each resident room. At the resident's expense they may have phone service installed in their room (hook-ups are available).

It is unfortunate that the phone was misplaced, but again we are not responsible to replace the phone for Mr. mother.

*** Sorry to break the news to you but your facility still smells of urine and feces. You never addresses the *** my mom, *** got at your facility and that she has gone through 2 bouts of *** upon dismissal.
