Refund for service consisting of upper and lower eye permanent eye liner makeup applied twice, not remaining permanent on eyes. On 6-25-15 Arch 2 Arch was paid $350.00 by a [redacted] check card to apply permanent makeup. I attended an appointment made to have a consultation concerning permanent eye makeup top,bottom,and eyebrows. Sales person,consultant andEsthetician [redacted] and I both agreed upon the appearance shown to me when she applied non-permanent eye pencil to the upper lid and lower eye line. I agreed to allow [redacted] to on 6-25-15 apply a what she explained a permanent eyeliner top and bottom of my eyes that would give me the same appearance permanently, as the eye pencil. After waiting 2 weeks as previously recommended by [redacted] to allow the makeup to blend itself to the skin/area makeup applied. I returned for a what she called a pretreatment/retouch which consisted of [redacted] reapplying the permanent makeup to the upper and lower eye. On 8-18-15 I made an appointment with [redacted] again advising her that the permanent makeup on my upper lid and lower eye didn't remain visible as applied on the two area's it was applied to. [redacted] advised me to come again, into the business location on [redacted], when asked cost for another application of makeup to be applied, [redacted] stated that the cost would be a charge of $75.00 stating the procedure would be a Touch up. I made the appointment with [redacted] yet didn't keep it due to the fact that the makeup didn't remain permanent after 2nd regular procedure, as guaranteed by her it would. Also an additional charge for a re-treatment. [redacted] had advised me on 7-9-15 that the procedure would last for 1 year before a touch up would need to be applied.Desired SettlementResolution to this is a request of $350.00 refunded immediately to my [redacted] Check Card Account or payment in the form of a check mailed to the address listed on this form. There are many reasons of not catching the pigment on the skin. For example if the client is performing yoga or exercising or doing a detox, the pigment would not stay since toxins are releasing from the body. There are few more reasons that this could happen. If client need more attention I would love to take care of her like I do all of my other clients. I would not let anyone be unhappy. So I would ask my client to give me a call at XXXXXXXXXX and make another appointment with me to do a consultation for free of cost.
