To Whom It May Concern:We are responding to a letter of complaint from a patient and are in need of some answers in regards to what is needed to take the next stepWe left a message with [redacted] today, in which we received a letter fromOn October 18, at 8:am, [redacted] came in for her regular periodontal maintenance and did a visual examDuring the exam, which was after the cleaning, no visual issues were foundThe patient was not due for x-rays due to having them done on 6/21/She then later called that afternoon, after 1:pm, to state that she noticed her bridge chipped after eating a burritoShe believes that it happened when hygienist was doing her cleaningWe attempted to have the patient come in that same day, but the patient stated unable to come in, so was scheduled for the next business day on 10/20/On this day, Dr [redacted] examined the patient in regards to her complaintAccording to Dr***, he noticed on tooth #a facial margin area of the porcelain crown chipped#13-Bridge is over years old and the defect area too small, not rough, no need to smoothen marginBridge still looks functional, but can be re-done at cost to the patient due to bridge being over years oldPatient seemed to understandAlso explained to patient that chip could not have been caused by cleaning, but can be re-done but charge will applyWe also took a photo, which we can also provide if neededPlease give us a call in regards to this matterThank you!Archibald Dental Office

for her regular periodontal maintenance and did a visual exam. During the exam, which was after the cleaning, no visual issues were found. The patient was not due for x-rays due to having them done on 6/21/2017. She then later called that afternoon, after 1:30 pm, to state that she noticed her bridge chipped after eating a burrito. She believes that it happened when hygienist was doing her cleaning. We attempted to have the patient come in that same day, but the patient stated unable to come in, so was scheduled for the next business day on 10/20/2017. On this day, Dr. [redacted] examined the patient in regards to her complaint. According to Dr. [redacted], he noticed on tooth #15 a facial margin area of the porcelain crown chipped. #13-15 Bridge is over 5 years old and the defect area too small, not rough, no need to smoothen margin. Bridge still looks functional, but can be re-done at cost to the patient due to bridge being over 5 years old. Patient seemed to understand. Also explained to patient that chip could not have been caused by cleaning, but can be re-done but charge will apply. We also took a photo, which we can also provide if needed.
