All the actions commited by the providers were reported to Ms. [redacted] and The Arizona State Deparment of Disabilities Mr.[redacted], we have decided to stop the service with this company due to their personal, we have records and substantial evidence to proceed legally but it doesnt worth. We won't do it. But in case the personal try to reach us again (as they did on March 24) we are working on a restriction with the Phoenix Police deparment. We simply dont want to know about them anymore.

Review: The contact person in this company [redacted] has failed to provide personal for the habilitation services for my son [redacted] in multiple times, in previous months she sent [redacted] who ask my wife for cash in order to keep the service and she also asked mi wife for her signature on the hours report where she had added extra time without being worked explaining that she would share money, then [redacted] was sent and she was continuously changing the arrival time last minute and lately she took my son to the park, without consulting and without answering her cellphone she took an extra one hour and 45 minutes a call was made to the police and when she showed up after that time she said she didn't have phone, four days later without a previous notice she show up to ask the signature the hours report and said she wasn't able to provide the service anymore. Since Friday March 14 my son does not have a provider.Desired Settlement: We require that Ms. [redacted] send a responsible person to fulfill the schedules.

Hello there,

This is in response to Complaint # [redacted] for Arion Care Solutions, LLC in Chandler Arizona.

Upon research of the issues presented by [redacted] I have discovered that [redacted] denies categorically all allegations that she asked for a signature for hours that she did not work. She worked all hours as arranged with the family. When the family first called on about this, we investigated and found that the hours were previously arranged with the family and the hours worked were the hours asked for. The second provider, [redacted], also worked exactly as asked. She did not change hours or ask for the family to commit fraud on her time sheet. There was a day when [redacted] forgot her phone. On that day the family did call the police, but [redacted] came back home before they arrived and the incident was reported. That incident was also investigated by our agency, and was resolved, we thought, to the satisfaction of all involved, including the family. WE believe that the family is only brining it up here as an attempt to complain about our staff trying to get paid hours they worked. [redacted] did quit as the family was inconsistent and making it very difficult to provide service. ON the day we heard that [redacted] couldn’t work with this family anymore, we spoke to the family to ask if they wanted us to send another provider out. The family was the one who said they did not need anyone. We offered staff [redacted] but the family closed out the service hours so we no longer serve this family. We cannot provide services to a family that has closed out our service. This is not our doing it is the family’s doing.

In our investigations, from the beginning, we found that the family was asking for hours and then trying to deny them when it came time to sign the time sheets. We worked with them on this and tried to send out other staff, and they attempted the same thing with other staff. Our staff left them because they were inconsistent. The staff that these complaints are alleged against are very upset and willing to write letters to provide additional evidence of their innocence.
