*** had her appointment on prior to her appointment , Arizona Heights Dentistry confirmed that I would have zero out of pocket cost for the procedure, sealantson the day of the appointment , the confirmed with me again that my out of pocket cost would be zerothis made sense since I gave them my correct insurance, delta dental of az I never gave inurance informationwe changed in 2016.I t is unethical when you quote a customer a pice of zero out of pocket and bill them for I was contacted by their office initially and thought I did owe the money then I realized that I did not after questioning them further.I did not receive numerous letters in the mail, except for the certified one recently sent to me.I t seems to me that this business is more focused on proving themselves to be right than repairing damaged relationships.I just think that when a business, any business we gives a customer a quote, they need to stand by it as closely as possible A descrepancy of $is unacceptable . [redacted] brought her 11 year old daughter [redacted] into the off ice for treatment on 12-8-2015. Prior to her appointment, we called Delta Dental of AZ to verify her benefits. Our office was told at that time [redacted] was cove red for sealants at 100% up to age 18 because it was preventative. We relayed this...

information to Amy and she decided to move forward . This became the start of the conflict. [redacted] provided us with the incorrect information. [redacted] statement regarding filing of the wrong insurance company is false. She said her insurance was [redacted]. We verified through [redacted] and her coverage did not start with them until Jan 12016.   We went a certified letter to end the patient/ doctor relationship due to nonpayment of fees after exhausting all efforts to contact [redacted] . We made multiple phone calls, left messages without a response. We sent multiple letters without any response. We sent a 14 day letter to contact us letting her know that if we did not hear from her, s he will be sent to collections. After months of trying to contact the patient, we gave up. We sent a certified letter to [redacted],the head of household and subscriber, informing that the family has been dismissed from t he practice and has been sent to a collect ion agency. I do not keep patients w ho do not pay their bills.   Dental insurance companies NEVER GUARANTEE payment until t he claim is sent after treatment is complete. We do our best to provide patients with the most accurate estimate but we cannot guarantee. [redacted] is ultimately responsible for a ll out of pocket costs for her daughter's treatment regardless of her insurance coverage outcome . I have included our financial policy that [redacted] signed and dated that she agrees to the terms o f our office policy. It states clearly that balances that are past due are subject to involvement with a collection agency. It is bold faced in the middle of the page,not in fine print. Amy accused me of being unethical. We have not done anything unethical.We recommended sealants for [redacted]. We called her insurance at the time, [redacted], to find out what coverage she had. They told us she would be covered 100%. Treatment was completed. The total of the 4 sealants was $180. The insurance paid $104. The remaining $76 is [redacted] responsibility. We billed her for the balance. On 4 -7-2016, Amy called to cancel a cleaning appointment for [redacted]. She said at that time that she will send payment. After this, we never heard from her, never received the payment although we continued collection efforts. Finally, her family was dismissed and sent to collections.   I feel I am not a t fault. My employee was courteous to call her insurance company for [redacted] behalf . have no control over what the insurance will pay or not pay. [redacted] should have contacted us months ago. She chose to ignore our efforts to contact her until she received the certified letter. I am requesting for Revdex.com to retract and dismiss this case.
