I went to this office for my cervical spine through TriWest Healthcare Alliance
They first require you to fill out ten pages of information about your self, that they send you in the mailthen when you get to the office they require you to fill out a additional pagesThen you site for almost a hour, and then they call you back to a desk just behind the door that you enteredI was then told in a very unpleasant and rude tone of voice by the female staff member, that they needed to take a photo of my to put on there computer, and that it was required that they do so, and it was not a option at all, and if you refused to do so, then you would not be seen at all!
I refused to have my photo taken, and I told the woman whyI said to her that you already made a copy of my Veterans ID card and that I did not want my photo and my medical information put together on there unsecure hard drive
She stated that it was office police, and that I should agree to disagree with her about the fact that she felt that my information was secure, and that I did not at all feel that waySo I told her that I would not agree to disagree at allI then asked to speak to her office manger, and was asked to go to the waiting room, where I waited minutes before the office manger came outI went again in behind the door to the desk, and she stood there with a very nasty look on her face
I explained to her what happened, and she asked me to give her the name of the person who I hade talked withI said to her that she is sitting behind her
She look over at her, and then back at me, and said, well what is the problem
I told her that it had to do with them taking a photo of me, and putting it on there hard drive, and that they already had my photo ID copied
She then got a even more unpleasant look on her face, and the tone of her voice also got even more nasty then the woman that I was complaining about in the first place
She then stated to me in a extremely rude tone of voice, that I would not be seen by the doctor without a photo !
I would never return to this office ever again for any reason !
Who know if what they do with your photo, and information, or where it is kept, or if it is at all secure in any wayI do not trust any one with all that information, considering all the identity theft these days !

My name is [redacted], I was operated on by Dr. Byron [redacted] W[redacted] MD I use MD lightly. I just received a letter from the AZ Medical Board saying no violations where found.
I will appeal this decision that took almost 1 year. They kept telling me it was hard to find a Dr that would review my case because doctors do not like to turn in other doctors.
Dr. W[redacted] put me through hell. He was not compassionate at all. I do believe part if this was because I'm gay. I had a supposed simple small brain tumor, I had the 1st operation was out in 2 days a Sunday. By Tuesday afternoon I was non responsive was taken to [redacted] on [redacted]. they then took a MRI said I had a bad infection and swelling in my brain. They called Dr W[redacted] they did not operate till the next day. I found out I had a infection from the 1st surgery that could have only received in the O.R. When I was being wheeled into O.R. I remember seeing huge screens on the wall but they had a rapper on all the screens no sound just the rapper. The surgical staff was discussing if the rapper was [redacted]. Then the wrong surgical tools arrived. After the surgery I was put into icu for 9 days. I only saw Dr. W[redacted] once after the surgery. The other days I was seen by his staff and PA. She was very incompetent. 2 weeks after I got out I had a appt with Dr. W[redacted] to get my staples out my partner and I stated all the issues we had with him, he listened all he said was, well that's in the past and you need put this whole experience behind you. There's nothing I can do now. That's when I decided to write a complaint to AZ medical board. But they turned me down not to surprised so I will appeal and do what I can. Iam far from done with Dr. W[redacted] MD
