When I saw the Arelo Eye Associates in I was never charged $for having my eyes checked and receiving a new prescription for glasses as it is stated from the Arleo Eye associates when they responded to your letter. NO Drhas ever charged me for a prescription of any kind. Thank you for your time and help in this matter*** ***
The $30 is for CPT 92015 - refraction. Because every patient does not want a new prescription, we only perform this test on patients that do. They they then incur the fee for the test performed to acquire the prescription. Some doctors bundle this into their exam fee and perform...
the test on all patients, we do not. It is, in fact, illegal and unethical for us to charge a patient for a piece of paper. This patient did pay this fee the last time she was in our office in 2010.

allowed amount according to their plan. THAT amount we do not know until the claim is submitted and paid (or not paid). The amount charged is our fee, we know that and can disclose it, but it has no bearing on anything, the patient pays what their insurance ALLOWS, if we participate. We do participate with her insurance, just not her vision plan. They are separate. We do not charge for the prescription. That is, in fact, illegal. The process of determining the prescription, the refraction, is a service with its own code and not covered by most insurances. Many patients do not want a new prescription, so we only do a refraction and charge the fee for those that do. The cash pay fee includes the refraction. I'm not sure why this patient feels she was somehow duped. Indeed she could have canceled her appointment and gone somewhere where her vision plan was accepted. She did not mention her vision plan when she scheduled her appointment. Since the majority of patients do not have a vision plan, it's not information we inquire about as a matter of course.
On 12/21/10 this patient wrote us a check, number [redacted], for $30.00. This was for her refraction, CPT 92015, that is a non-covered service. On January 10, 2011, after her insurance had been billed for her exam, we sent her a bill for the portion her insurance said she was responsible for. She sent us check number 2604 for $45.43 for CPT 92004. We can send this patient a printed copy of her ledger history if she'd like. Or she's welcome to come in the office and have someone review it with her.
