Review: I went to Arlington Center for Dentistry on 24 March for tooth sensitivity in tooth 31. The dentist put in a temporary crown on for which I paid 299.75. The temporary crown worked just fine, no issues, I was able to eat hard foods. On 7 April, I returned to the dentist to get the permanent crown and the dentist claimed it did not fit correctly so he had another crown created which took over two weeks. On 24 April, I returned to get the new crown put in and the dentist rushed everything. He did not wait for me to get numb enough, and shoved the crown on my tooth rushing the whole job. You can physically see a dark blood line on my tooth where my gum was bleeding when he put the crown on. I paid another 299.00 upon getting the permanent crown. A few weeks later I still have tooth sensitivity from the botched up dental work. I have been in the habit of chewing on the less sensitive side of my mouth and avoiding hard foods which is why I didn't immediately notice my tooth sensitivity with the permenant crown. I want a full refund for lack of adequate services.Desired Settlement: Full refund for lack of services and a letter sent to my insurance company [redacted] stating exactly what occurred and why the crown had to be refunded.

July 3, 2014Dear Madam/Sir:This letter is to clarify the dispute explanation for the above-referenced case in the amount of $600.00 refund requested by **. [redacted].Dr. [redacted], a dental provider for Arlington Center lor Dentistry, provided a two appointment treatment to **. [redacted] starting on March 30, 2014 and completed on April 24, 2014.**. [redacted] signed consent forms for treatment, financial agreements to fees associated with the dental services recommended, in addition to a signed receipt for the authorization charge on her [redacted] credit card and submitted to her [redacted] Insurance on April 24, 2014. The credit card was present and was swiped utilizing our merchant terminal. Copies of the aforementioned documents and receipt are available, if needed.**. [redacted] telephoned our office on May 18, 2014, expressing discomfort and sensitivity to her permanent crown, three weeks after her last visit on April 24, 2014. The treating dentist, Dr. [redacted], immediately returned her call but could not reach her directly. He left a voicemail message on her phone asking her to return to the practice so he could evaluate her symptoms and make adjustments as necessary.On May 19, 2014, [redacted] called back at 5:27 PM while Dr. ** was in treatment. Although the receptionist informed her that Dr. ** would be with her momentarily, **. [redacted] chose not to wait. Instead, she told the receptionist to relay to Dr. ** of her dissatisfaction and displeasure with the pain and sensitivity associated with her crown, and that she had no intention of returning to the practice for further evaluation, adjustment, and/or treatment. She did not want to wait to speak directly to Dr. **.Our practice prides in providing the best care for our patients, we firmly believe in doing our best to ensure that all our patients, including **. [redacted], receive good care with our full attention. We stand behind our quality of care and practice standards. Dr. ** completed treatment of services to **. [redacted]. Dr. ** asked **. [redacted] to return for follow-up care so he could address her concerns, but she refused. It is unfair of **. [redacted] to withhold payment for services rendered.Further, it is unfair of **. [redacted] to withhold payment without giving us a fair opportunity to resolve her dissatisfaction. Crown adjustments are not an unusual occurrence in dentistry, and we are confident that Dr. ** can provide the necessary adjustment to **. [redacted]'s satisfaction.At this time, we elect to refrain from providing a refund. Please feel free to contact me for additional information.
