July 19, 2016Revdex.comPO Box 1000Dupont, WA 98327ATTN: [redacted] RE: ID # [redacted] Ms [redacted] ,The following is in response to your letter dated June 14, 2016.Due to HIPPA law I will reference patient’s a Patient A and Patient B.Patient A entered our clinic 02/05/and her last visit was 05/18/ Patient A was seen a total of timesEach time the patient comes into our office they fill out a pain rating sheet which Patient A didAfter the patient is seen there chart comes back to the front desk and the services that were performed are entered into our systemPatient A’s did not have chiropractic benefits on her insurance plan, therefore, was put on a cash pay basisShe was set up on a monthly payment planOut of the months of care, Patient A only paid of the months.Patient B entered our clinic 02/17/and his last visit was on 05/08/Patient B was seen a total of timesPatient B did have chiropractic insurance benefits and asked that we bill his insurerOn Patient B’s second visit to our office, it was explained to him that he had a high deductible that was still needing to be metHe understood that and still continued to come in for treatment.Both patients’ were sent several statements as well as telephone messages with no responseTherefore, these accounts were turned to collectionHad either patient contacted our office regarding their accounts, a resolution would have been done.To resolve this matter we have removed them from collection and any remaining balance has been written off.Should you have any further questions, please feel free to contact me.In Health,Shawnna F*Office ManagerArlington Spine Center(360) 474-

Our company spent long hours with the complaints husband on design and redesign of a custom buildingUpon execution of contract many things were changed and redoneI spent many hours with him to get him deals on doors ect Our company spent weeks there helping with excavator and trenching
and drilling holesWe had to modify existing building to accept new building as well as use our equipment to finish the leveling processUnfortunately the weather turned extremely cold and froze everythingMr*** then said come back in spring Come March we were getting ready to get going on it and I had spoken to him we were finishing a job and would start approx weeks later his son called Me and said dad is sick and we don't wanna build the buildingI stated we had a contract and I was entitled to the profit of the jobHis father then passed and we have been going round and round on this buildingThey even stopped at a customer job weeks agoThe profit on this job was to be approx 17,I have plenty of man hours in this job as well as equipment time ectWe would have gladly built this building but they have made it clear the farm will be going away and there is no need for it now

Patient A was seen a total of 21 times. Each time the patient comes into our office they fill out a pain rating sheet which Patient A did. After the patient is seen there chart comes back to the front desk and the services that were performed are entered into our system. Patient A’s did not have chiropractic benefits on her insurance plan, therefore, was put on a cash pay basis. She was set up on a monthly payment plan. Out of the 4 months of care, Patient A only paid 3 of the months.Patient B entered our clinic 02/17/2016 and his last visit was on 05/08/2016. Patient B was seen a total of 14 times. Patient B did have chiropractic insurance benefits and asked that we bill his insurer. On Patient B’s second visit to our office, it was explained to him that he had a high deductible that was still needing to be met. He understood that and still continued to come in for treatment.Both patients’ were sent several statements as well as telephone messages with no response. Therefore, these accounts were turned to collection.
