correct one over the phone. I apologize again that you had such a hard time with that issue, but the reason we were unable to take the information over the phone is because the insurance plan you have provides debit cards as a member insurance card and we must always have a copy on file prior to submitting a claim. I understand we had requested a copy, and you had agreed to fax the documents. I am not sure why we were not receiving the fax initially, but I do assure you we did receive it on 03/30/17 and have since spoke to your insurance to get the issue resolved. I also have taken the steps to provide additional training for our billing staff to provide better communication to our patients.
