Pt presented on 4-30-to the Emergency Department c/o low back pain s/p MVA about two hours agoShe was triaged and brought back into the treatment area and was seen and treated by a physician from Middlesex Emergency AssocThe patient had x-rays taken and was given medicationThe patient
signed both general consent and billing consent agreeing that she may get separate bills from both the Emergency room physician (balance of $is from them) and from Edison Radiology Group, PA for the reading of any radiology services ordered while in the Emergency Room (balance of $is from them) These are both physician charges not facilityThe facility charges include charge for use of the ED and for the use of xray equipment and for the technician taking both cervical and lumbar xraysWe were paid per CHN pricing of 75% per the MV fee schedule with a patient responsibility of $per pt's Motor Vehicle coverage with AAAThe patient can request a copy of her medical records and a copy of her bill by contacting *** *** ***The charges are not redundant as we billed for the facility and the physicians bill separately for their servicesOur prices are comparable to other area hospitalsThe patient needs to contact Edison Radiology billing services with any questions regarding out-of-network issuesThey can be reached at ***

Yesterday when I read JFK's message, the only word that can describe my feeling is anger. JFK's message was totally lying. It said :( patient ) "was seen and treated by a physician from Middlesex Emergency Assoc", the fact was a guy measured my blood pressure, that was the only thing they did beside X-ray. I don't know whether that guy was a physician or not, but it is hard to rate it as a treatment and charge extra fee beside the $they charged my insurance company.( patient) "was given medication", the fact was I didn't take any medication from them. Again they charged extra $

Regarding my complaint [redacted],  it had been resolved and I amsatisfied with the business' response.  Please update the status.

Review: I have a complaint to register with the Arthur Murray Dance Studio in Wexford, owned by [redacted] I had prepaid for some dance lessons and had accumulating knee problems. When I signed my original contract I was told that all unfinished lessons could be bought back by the company or my unused lessons would be refunded in full I have dealt with trying to get my refund for over 15 months now. I have spoken with my original instructor, [redacted] on MULTIPLE phone calls. I have been transferred back and forth, put on hold, promised a refund, delayed and lied to. I get very negative impressions of this company from the way I have been treated after paying in advance in good faith at this establishment. The business is rated F on the [redacted] forum and this worries me. I found out that [redacted] left an Ohio studio and didn’t refund those dance students money they had paid. Something is seriously wrong with the studios he franchises. All I want is my remaining money back that they have under my name. I have no idea why I am getting the run around every time I call in and speak to their staff.Desired Settlement: see Attached document

[redacted] enrolled on lessons in July 2011. According to her enrollment agreement those lessons were to be completed by November 2011 and after that date were not eligible for refund. Naturally, we refund any lessons at anytime and for any reason during the eligible time frame. As we often do as a courtesy to our students, we continued to teach her lessons after that expiration date through April of 2012. She called the studio a month ago asking for a refund but was explained that we are a Franchise and cannot deviate from her signed agreement. We did, however, find a student willing to purchase those last couple of lessons from her, but she turned that offer down for some reason?? Our studio is rated A+ by the Revdex.com so I have no idea what she is referencing in the last half of her comments. Again we would be willing to work with her on selling those last lessons even though it's 3 years later. [redacted] would just need to write us a letter explaining how she would like to proceed, again as detailed in her enrollment agreement. Thank you for the time.
